Additional tests may be recommended as a part of the health exam based on individual needs. The patient is responsible for any charges not covered by their health insurance.

Annual membership fees paid by you are solely for the Amenities listed on this website that are furnished, or by the LLC. All Professional Services are provided by PartnerMD^®, PC, which engages your physician to provide medical services to you. No Professional Services are provided by the LLC. The membership fee does not pay for any Professional Services or for any items or services covered by your health plan. The PC participates with most major health plans and will seek payment from your health plan for covered Professional Services. The PC will accept payment from your health plan as payment in full for covered Professional Services, subject to applicable deductibles, co-payments and co-insurance amounts. Some Professional Services may only be covered in part by your existing health plan. Check your health plan's schedule of benefits for coverage information.
