What We Gather From You

PartnerMD gathers information on our website in order to provide relevant information to you, explain our services and improve the user experience on our website. We do not intend to obtain any personal information from minors. If you are under the age of 18, please do not provide PartnerMD with any personal information.

PartnerMD may gather two types of information from you from this website:


Personally Identifiable Information (PII)

Our “Get Started” form and other forms on the website may request information that you choose to disclose to PartnerMD. Examples of PII include your name, email address, phone number, address or any other information that can be used to identify you.

PartnerMD may collect and use PII to:

If you do not want to provide PII through our website, you always have the ability to call one of our locations directly to provide information over the phone to help us fulfill your request. We may gather additional information that may or may not be traced specifically back to you. At this time, PartnerMD does not request highly sensitive information, such as Protected Health Information (PHI), credit card information, or social security numbers, on our website. Our website contains links to other websites, including a link to our patient portal and wellness tracking software. These links are provided on our website for your convenience. The privacy policy stated on this website does not extend to any external websites. You should familiarize yourself with the privacy policies of any external websites that you visit.


Social Media Features and Widgets

Our website includes social media features and widgets, such as the Facebook Like button or other interactive mini-programs that run on our site. These features may collect personal information such as your email address, and they may collect information such as your IP address and which page you are visiting on our site. They may set cookies to enable certain functions. Social media features and widgets are either hosted by a third-party or hosted directly on our website. Your interactions with these features are governed by the privacy policies of the companies providing them.


Chat Rooms, Message Boards, Testimonials and Public Forums

Please keep in mind that whenever you voluntarily disclose personal information online – for example on message boards, through e-mail, or in chat areas – that information can be collected and used by others. SMS Privacy Policy

PartnerMD may collect your phone number via forms on our website or event registrations and use your phone number to:

  • Contact you via phone call or text message to provide more information about PartnerMD, our services, special promotions or programs of interest and to perform customer service functions.

When opted-in, you will receive text messages (SMS/MMS) to your mobile number. These kinds of messages may include a response to a form completion, real-time texts to ask and answer questions about our services and pricing, event invitations, feedback requests, project updates, follow-ups.

Message frequency will vary.  You can opt out of this service at any time. Just text “END” to the phone number. After you text “END” to us, we will send you an SMS reply to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just text “START,” and we will start sending SMS messages to you again.

Emails

We may use a third-party vendor to help us manage some of our email communications with you. While we may supply this vendor with email addresses of those we wish them to contact, your email address is never used for any purpose other than to communicate with you on our behalf. We never share any information, other than your email address, with our third-party email vendor, which is obligated not to share these email addresses with anyone else.

Even if you have given us permission to send emails to you, you may revoke that permission at any time by following the unsubscribe information at the bottom of each such email.

When communicating via email directly with a PartnerMD representative, is it important to remember that an email message is unsecured. Please exercise caution before providing sensitive information via email. You may always choose to provide sensitive information by postal mail or phone.


Privacy of Medical Information Exchanged as a PartnerMD Member

The privacy policy on this website pertains only to your interactions with PartnerMD through the website. PartnerMD members receive a separate privacy policy that details how PartnerMD handles the privacy of your medical information obtained as part of membership and interactions with PartnerMD staff regarding your medical care, as well as your rights with respect to the access, use and disclosure of PHI under HIPAA.


Opting Out

If you do not wish to receive any marketing information from PartnerMD, you may request to be placed on our “Do Not Mail” list by making a request using the form on our Contact Us page. PartnerMD may require additional PII to properly record your request.


Changing Your Information

If you would like to change the information you’ve provided through our website, you may do so by calling our membership inquiry line or emailing our info email address for the location nearest you. Contact information for each location can be found on the Contact Us page. You will need to provide the PII that you would like to change first so PartnerMD can correctly identify you.


Changes to This Policy

PartnerMD reserves the right to alter this policy at any time. If we modify this policy, we will notify users of this change by posting changes on the website. Your continued use of the website following a posted change to PartnerMD’s privacy policy will serve as your agreement to any new or deleted terms. Please refer to the date listed at the bottom of this page to determine when the last changes were made to this policy. If any changes are not acceptable to you, your only recourse is to cease using the PartnerMD website.
